Definition and Anatomy
The pterygomandibular space (Latin: Spatium pterygomandibulare) is a narrow anatomical compartment filled with loose connective tissue and fat, located in the region of the lower jaw. It is bounded medially by the medial pterygoid muscle and laterally by the ramus of the mandible. The superior boundary is formed by the lateral pterygoid muscle.
Anteriorly, the space communicates with the buccal space, while posteriorly it connects with the deep masticator space. The compartment is partially defined by the pterygomandibular fascia.
Contents of the Space
Several important anatomical structures pass through the pterygomandibular space:
- Inferior alveolar nerve: A branch of the mandibular nerve (V3, third division of the trigeminal nerve), which travels through this space before entering the mandibular canal.
- Lingual nerve: Passes through the space medially and anteriorly to the inferior alveolar nerve, providing sensory innervation to the tongue.
- Inferior alveolar artery and vein: Vascular structures accompanying the inferior alveolar nerve.
- Loose connective and adipose tissue that permits the spread of injected fluids.
Clinical Significance
Inferior Alveolar Nerve Block (Mandibular Block Anesthesia)
The most important clinical application of the pterygomandibular space is in inferior alveolar nerve block anesthesia, routinely performed in dentistry and oral and maxillofacial surgery. Local anesthetic is injected precisely into this space to anesthetize the inferior alveolar nerve, allowing pain-free procedures on the lower jaw teeth, mandibular bone, and surrounding mucosa on the corresponding side.
Correct identification of the space is critical for a successful block. Clinicians use the pterygomandibular raphe -- a mucosal fold running between the pterygoid hamulus and the lingula of the mandible -- as a key landmark to guide needle placement.
Spread of Infections
The loose connective tissue within the pterygomandibular space also facilitates the spread of infections. Odontogenic infections (originating from teeth), especially from lower wisdom teeth (third molars), can rapidly extend into this space, leading to a pterygomandibular abscess or phlegmon.
Typical signs and symptoms include:
- Trismus: Painful limitation of mouth opening due to involvement of the medial pterygoid muscle
- Difficulty swallowing (dysphagia or odynophagia)
- Swelling in the floor of the mouth or cheek region
- Fever and general malaise
Infection in this space is a medical emergency requiring prompt surgical drainage and systemic antibiotic therapy to prevent spread to adjacent spaces (e.g., the parapharyngeal space) and potentially life-threatening complications.
Relevance in Oral and Maxillofacial Surgery
In surgical procedures such as wisdom tooth removal, mandibular osteotomies, or tumor resection, the pterygomandibular space serves as an important anatomical reference. A thorough understanding of this compartment helps surgeons protect neurovascular structures and avoid complications.
