Ambroxol is an active metabolite of bromhexine and is one of the most commonly used over-the-counter medications worldwide.

Ambroxol is indicated for the following conditions:
- Acute and chronic bronchitis
- Common cold with mucus congestion
- Bronchial asthma (as supportive therapy)
- Chronic obstructive pulmonary disease (COPD)
- Cystic fibrosis (as adjunct therapy)

Mechanism of Action
Ambroxol works through several complementary mechanisms in the respiratory system:
- Mucolytic effect: It alters the composition of bronchial secretions by breaking the bonds between mucin proteins, resulting in thinner, less viscous mucus.
- Secretomotor effect: Ambroxol stimulates serous glands in the bronchial mucosa to increase fluid secretion, which dilutes the mucus.
- Ciliary stimulation: The substance enhances the activity of cilia (tiny hair-like structures) in the airway epithelium, promoting more effective mucus clearance toward the throat.
- Local anesthetic effect: At higher concentrations, ambroxol blocks sodium channels in nerve cells, providing a pain-relieving effect in the throat and mouth.
- Surfactant stimulation: Ambroxol promotes the production of pulmonary surfactant by type II pneumocytes in the lungs, reducing surface tension in the alveoli and helping to keep them open.
Dosage and Usage
The dosage of ambroxol depends on age, dosage form, and the condition being treated. Typical adult dosages include:
- Tablets/capsules: 30 mg, two to three times daily (extended-release capsules: 75 mg once daily)
- Syrup/drops: Dosed according to body weight and age, especially in children
- Lozenges/throat spray: For local pain relief in the throat area
- Inhalation solution: For direct application via nebulizer devices
It is recommended to maintain adequate fluid intake during treatment, as this supports the mucolytic effect. For self-medication without medical advice, the duration of use should not exceed 4–5 days.
Side Effects
Ambroxol is generally well tolerated. Possible side effects include:
- Nausea, vomiting, or gastrointestinal discomfort (especially at higher doses)
- Dry mouth or increased salivation
- Numbness in the mouth or throat (with local application)
- Rarely: allergic reactions such as skin rash or itching
- Very rarely: severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis) – seek immediate medical attention if these occur
Interactions and Precautions
When used in combination with certain antibiotics (e.g., amoxicillin, erythromycin), ambroxol may increase their concentration in lung tissue, which can be therapeutically beneficial. Concurrent use with antitussives (cough suppressants) should be avoided, as this may cause secretion buildup in the airways. Caution is advised in patients with impaired kidney or liver function.
