The primary goal is to save a tooth when standard root canal treatment has not been able to fully eliminate infection or inflammation at the root tip.
When is an Apicoectomy Needed?
This procedure is typically recommended in the following situations:
- A previous root canal treatment has failed to clear the infection at the root tip.
- A granuloma (a small mass of inflamed tissue) or a cyst has formed at the root apex.
- The tooth has a complex root anatomy that prevents complete cleaning from within the canal.
- An existing post, crown, or other restoration makes retreatment from the top of the tooth impractical.
- Persistent pain or swelling despite completed root canal therapy.
How the Procedure Works
An apicoectomy is typically performed on an outpatient basis under local anesthesia. The main steps include:
- Access incision: The dentist or oral surgeon makes a small incision in the gum tissue near the affected tooth and gently folds it back to expose the underlying bone.
- Bone access: A small opening is made in the bone to access the root tip and the surrounding infected tissue.
- Root end resection: Approximately 3 mm of the root tip is removed, along with the granuloma or cyst, using a surgical drill.
- Retrograde filling: The exposed end of the root canal is cleaned and sealed with a biocompatible material such as MTA (Mineral Trioxide Aggregate) to prevent reinfection.
- Wound closure: The gum tissue is repositioned and sutured closed.
Recovery and Aftercare
After the procedure, patients may experience mild swelling, discomfort, and tenderness in the treated area for a few days. The following aftercare guidelines are generally recommended:
- Apply a cold pack to the cheek for the first 24 hours to reduce swelling.
- Take prescribed or over-the-counter pain relief as directed (e.g., ibuprofen).
- Avoid chewing on the treated side and take care when brushing near the surgical site.
- Attend follow-up appointments so the dentist can monitor bone healing via X-rays.
- Avoid smoking and alcohol, which can impair healing.
Complete bone regeneration at the surgical site may take several months and is confirmed through radiographic follow-up imaging.
Success Rates
Apicoectomy is a well-established procedure with consistently high success rates. Studies report long-term success rates of 80–90%, especially when modern microsurgical techniques and magnification tools (such as a surgical microscope) are used. If the procedure does not result in healing, tooth extraction may be considered as a last resort.
Risks and Possible Complications
As with any surgical procedure, complications are possible, though they are uncommon:
- Post-operative bleeding or bruising
- Wound infection
- Temporary numbness or nerve sensitivity, particularly in the lower jaw
- Injury to adjacent teeth, sinuses (upper jaw), or nerves
- Failure to heal, resulting in recurrent infection
