Absorption enhancement refers to all strategies, measures, and formulation approaches designed to increase the absorption (resorption) of an active substance, nutrient, or compound from the gastrointestinal tract into the bloodstream. Improved absorption means that the body can utilize a greater proportion of the ingested substance. This is of great importance in both pharmacy and nutritional medicine.
Bioavailability describes how much of an ingested substance reaches the systemic circulation in an unchanged form. Absorption enhancement measures aim directly at increasing this bioavailability.
Causes of Poor Absorption
Reduced absorption can have many causes:
- Poor solubility: Many active substances are water-insoluble and therefore difficult to absorb in the aqueous environment of the intestine.
- Chemical instability: Some substances are broken down by stomach acid or digestive enzymes before they can be absorbed.
- Low membrane permeability: Certain molecules are too large or too polar to effectively penetrate the intestinal wall.
- Meal dependency: Some substances are only well absorbed in the presence of fats or other nutrients.
- Intestinal diseases: Conditions such as celiac disease, Crohn's disease, or a disrupted gut flora can impair absorption.

Strategies for Absorption Enhancement
Pharmaceutical Formulation Approaches
In pharmacy and the dietary supplement industry, various pharmaceutical (galenic) technologies are used to increase bioavailability:
- Nanoemulsions and liposomes: Active substances are embedded in tiny fat particles, which considerably improves their uptake through the intestinal wall.
- Micronization: Reducing particle size increases the surface area, thereby promoting solubility and absorption.
- Solid dispersions: Poorly soluble substances are distributed in a well-soluble matrix to increase their dissolution rate.
- Enteric coatings: Protects sensitive active substances from stomach acid, so they are only released in the small intestine.
- Cyclodextrin inclusion complexes: Active substances are enclosed in ring-shaped sugar molecules to improve their solubility.
Use of Absorption Enhancers (Bioenhancers)
Certain substances can actively promote the uptake of other compounds. These are referred to as bioenhancers:
- Piperine (from black pepper): One of the most well-known examples. Piperine inhibits certain metabolic enzymes (e.g., CYP3A4) and transport proteins in the intestinal wall, significantly increasing the absorption of many substances. For instance, it increases the bioavailability of curcumin many times over.
- Vitamin C: Promotes the uptake of non-heme iron from plant sources by converting trivalent iron into the more readily absorbable divalent form.
- Quercetin: A plant compound that can inhibit the activity of certain efflux pumps, thereby improving the absorption of some active substances.
- Fats and oils: Significantly increase the absorption of fat-soluble vitamins (A, D, E, K) and other lipophilic substances.
Dietary Measures
The correct time of intake and combination with certain foods also play an important role:
- Fat-soluble vitamins should be taken together with a fatty meal.
- Iron is best absorbed on an empty stomach or together with vitamin C.
- Calcium and iron should not be taken at the same time, as they mutually inhibit each other's absorption.
- Magnesium is better absorbed in organic compounds (e.g., magnesium citrate) than in inorganic forms (e.g., magnesium oxide).
Improved Dosage Forms
The type of dosage form significantly influences absorption. Liquid preparations (e.g., oral solutions, drops) are generally absorbed more quickly than solid forms (tablets, capsules). Sublingual (placed under the tongue) or transdermal (absorbed through the skin) dosage forms bypass the digestive tract entirely, which can dramatically increase bioavailability for certain substances.
Clinical Relevance
Absorption enhancement is of great importance in clinical practice. It enables:
- A reduction in the required dose, saving costs and minimizing side effects.
- A more reliable and predictable effect of medications.
- The effective treatment of patients with absorption disorders (e.g., after intestinal surgery).
- Better utilization of nutrients from food and dietary supplements.
