What is Acrocyanosis?
Acrocyanosis is a chronic disorder of the small blood vessels (arterioles and capillaries) in the extremities – primarily the hands, feet, and occasionally the face. Due to persistent narrowing of these vessels, oxygen-rich blood is reduced to the outermost parts of the body, resulting in a characteristic bluish-violet discoloration of the skin. Unlike other circulatory disorders, acrocyanosis typically does not cause pain.
Causes
The exact cause of primary acrocyanosis is not fully understood. Two forms are distinguished:
- Primary acrocyanosis: Occurs without an identifiable underlying disease, most commonly in young, slim women. It is caused by a functional hypersensitivity of the small blood vessels to cold temperatures.
- Secondary acrocyanosis: Develops as a result of an underlying condition, such as eating disorders (anorexia nervosa), connective tissue diseases, vascular disorders, bone marrow conditions, or certain medications.
Contributing factors include cold exposure, stress, and low blood pressure.
Symptoms
Typical signs of acrocyanosis include:
- Bluish-violet discoloration of the fingers, toes, hands, and feet
- Cool, moist, and often excessively sweating skin in affected areas
- Swelling (edema) of the hands and feet, particularly in warmth
- Worsening of symptoms in cold conditions, improvement with warmth
- Generally no pain and no tissue damage
Unlike Raynaud syndrome, the discoloration in acrocyanosis is persistent rather than occurring in episodes.
Diagnosis
Diagnosis is primarily clinical, based on physical examination and medical history. The physician looks for:
- Characteristic appearance of the affected skin areas
- Exclusion of other conditions such as Raynaud syndrome or secondary causes of acrocyanosis
Further investigations such as capillary microscopy (examination of the smallest blood vessels under a microscope), blood tests, and vascular measurements may be performed to rule out a secondary form.
Treatment
In primary acrocyanosis, specific treatment is usually not required, as the condition is benign and does not cause harm. The following measures can relieve symptoms:
- Warmth and cold protection: Wearing warm gloves, socks, and appropriate clothing
- Regular physical activity to promote circulation
- Avoiding nicotine, as smoking worsens vasoconstriction
- In severe cases, vasodilatory medications (e.g., calcium channel blockers) may be prescribed
In secondary acrocyanosis, treatment of the underlying condition is the primary goal.
When to See a Doctor?
Although acrocyanosis is usually harmless, a doctor should be consulted if the discoloration is new, painful, worsening, or accompanied by other symptoms such as weight loss, fatigue, or joint problems. This could indicate a secondary form with an underlying condition requiring treatment.
