What Is an Amoebic Abscess?
An amoebic abscess is a serious complication of infection with the single-celled parasite Entamoeba histolytica. It results in the formation of a pus-filled cavity (abscess) within an organ, most commonly the liver (hepatic amoebic abscess). Less frequently, the lungs, brain, or other organs may be affected. Amoebic abscess is one of the most common serious parasitic diseases worldwide and is particularly prevalent in tropical and subtropical regions.
Causes and Transmission
The causative agent, Entamoeba histolytica, is transmitted through the fecal-oral route, typically via contaminated drinking water or food. After ingestion, the parasite first colonizes the large intestine and can then migrate through the bloodstream to the liver, where it causes tissue destruction and abscess formation. Key risk factors include:
- Travel to or residence in tropical regions (South and Central America, Africa, India, Southeast Asia)
- Consumption of untreated water or unwashed raw fruits and vegetables
- Poor sanitation and hygiene conditions
- Immunosuppression or weakened immune system
- Middle-aged men are disproportionately affected compared to women
Symptoms
Symptoms of an amoebic abscess may develop gradually or appear suddenly. Common signs include:
- Right upper abdominal pain, often radiating to the right shoulder
- Fever (frequently above 38.5 °C / 101.3 °F) and chills
- General malaise, fatigue, and weakness
- Nausea and unintentional weight loss
- Enlarged and tender liver (hepatomegaly)
- Cough (if the abscess extends toward the lung)
Notably, diarrhea or other bowel symptoms are not always present, as a liver abscess can develop in the absence of active intestinal disease.
Diagnosis
Diagnosis of an amoebic abscess typically involves a combination of imaging studies and laboratory tests.
Imaging Studies
- Ultrasound: The first-line imaging method for visualizing liver abscesses
- CT scan or MRI: Used for more precise localization and assessment of the abscess
Laboratory Tests
- Serology: Detection of antibodies against Entamoeba histolytica in the blood (highly sensitive and specific)
- Elevated inflammatory markers (CRP, white blood cell count, ESR)
- Elevated liver enzymes (transaminases, alkaline phosphatase)
Direct Pathogen Detection
- Stool examination for amoebic cysts or trophozoites
- PCR testing for specific identification of Entamoeba histolytica
- Aspiration of the abscess (reserved for unclear diagnoses or failure to respond to treatment)
Treatment
Amoebic abscess is treated primarily with medication and responds well to appropriate therapy in the majority of cases.
Drug Therapy
- Metronidazole is the first-line treatment and acts against tissue-invasive amoebae. It is typically administered orally or intravenously for 10 days.
- Following metronidazole, a luminal amoebicide such as paromomycin or diloxanide furoate is prescribed to eliminate residual amoebae in the intestine and prevent relapse.
Abscess Drainage
- Percutaneous drainage or needle aspiration is indicated when the abscess is very large (greater than 5 cm), does not respond to medication, or carries a risk of rupture.
- Surgical intervention is rarely required and is reserved for complicated cases.
Prognosis
With early diagnosis and appropriate treatment, the prognosis is generally excellent. If left untreated, the abscess can rupture into the abdominal cavity, pleural space, or pericardium, leading to life-threatening complications.
Prevention
Prevention relies on strict hygiene measures, especially when traveling to high-risk areas:
- Drink only boiled or properly filtered water
- Thoroughly wash and peel fruits and vegetables
- Avoid raw or undercooked food in high-risk regions
- Practice regular and thorough handwashing with soap
Currently, no vaccine against amoebic infection is available.
