What Is Autoimmune Therapy?
In an autoimmune disease, the body's immune system mistakenly attacks healthy tissue, identifying it as foreign or dangerous. Autoimmune therapy refers to all medical treatment strategies aimed at controlling, dampening, or specifically regulating this faulty immune response. There are more than 80 known autoimmune diseases, including rheumatoid arthritis, multiple sclerosis, type 1 diabetes, systemic lupus erythematosus, and Hashimoto thyroiditis.
Goals of Autoimmune Therapy
The main goals of autoimmune therapy include:
- Suppression or modulation of the overactive immune response
- Reduction of inflammation and associated symptoms
- Prevention of permanent organ damage
- Maintenance or improvement of the quality of life
- Achieving remission (a symptom-free phase)
Types of Therapy and Mechanisms of Action
Immunosuppressants
Immunosuppressants are medications that broadly reduce the activity of the immune system. They are commonly used in severe autoimmune conditions. Well-known agents include methotrexate, azathioprine, and mycophenolate mofetil. Because they weaken the entire immune system, they increase the risk of infections.
Corticosteroids
Corticosteroids such as prednisone or prednisolone have potent anti-inflammatory and immunosuppressive effects. They are often used short-term during acute flares, as long-term use can cause significant side effects including osteoporosis, weight gain, and high blood pressure.
Biologics
Biologics are biotechnologically manufactured drugs that intervene specifically in targeted signaling pathways of the immune system. They block certain messenger substances or immune cells involved in the inflammatory response. Examples include TNF-alpha blockers (e.g., adalimumab, etanercept), as well as interleukin inhibitors and B-cell inhibitors. Biologics have significantly improved the treatment of many autoimmune diseases.
JAK Inhibitors
JAK inhibitors (Janus kinase inhibitors) are a newer class of orally administered medications that block signaling pathways inside immune cells. They are used in conditions such as rheumatoid arthritis and inflammatory bowel disease, among others.
Plasmapheresis
Plasmapheresis is a procedure in which blood plasma is filtered to remove harmful antibodies. This method is used in certain severe autoimmune diseases such as Guillain-Barré syndrome and myasthenia gravis.
Intravenous Immunoglobulins (IVIG)
Intravenous immunoglobulins are concentrated antibody preparations derived from donor blood. They modulate the immune response in complex ways and are used in various autoimmune and inflammatory conditions.
Tolerance Induction and Immunomodulatory Approaches
Newer research approaches aim to retrain the immune system to recognize the body's own structures as harmless, a process called tolerance induction. These include antigen-specific immunotherapy and the use of regulatory T-cells. Some of these approaches are still in clinical trials.
Diagnosis and Treatment Planning
Before initiating autoimmune therapy, a comprehensive diagnostic workup is performed. This includes blood tests (e.g., for autoantibodies such as ANA, anti-dsDNA, or rheumatoid factor), imaging procedures, and in some cases tissue biopsies. The choice of therapy depends on the specific disease, its severity, the organs affected, and the patient's general health status.
Side Effects and Risks
Since most autoimmune therapies weaken the immune system, there is an increased risk of infection. Other potential side effects vary depending on the agent used and may include:
- Increased susceptibility to infections (bacterial, viral, fungal)
- Increased risk of certain cancers over the long term
- Liver and kidney dysfunction
- Changes in blood count
- Gastrointestinal complaints
Regular medical check-ups and laboratory monitoring are therefore an essential component of any autoimmune therapy.
Lifestyle and Supportive Measures
In addition to drug therapy, the following measures can support treatment success:
- Balanced, anti-inflammatory diet
- Regular, adapted physical activity
- Stress reduction (e.g., through relaxation techniques such as yoga or meditation)
- Adequate sleep and rest
- Avoidance of smoking and excessive alcohol consumption
- Psychosocial support and patient counseling
