What Does Analgesic Mean?
The term analgesic derives from the Greek words an- (without) and algos (pain). It describes the ability of a substance, drug, or therapeutic method to relieve or completely eliminate pain without necessarily affecting consciousness. Substances with this property are called analgesics and are among the most widely used medications worldwide.
Mechanism of Action
Analgesic substances interfere with pain perception at various levels of the nervous system. The mechanisms differ significantly depending on the drug class:
- Peripheral analgesics (e.g., NSAIDs such as ibuprofen, aspirin): Inhibit the enzymes cyclooxygenase-1 and -2 (COX-1/COX-2), thereby reducing the production of prostaglandins – inflammatory mediators that sensitize pain receptors in tissue.
- Centrally acting analgesics (e.g., opioids such as morphine, tramadol): Bind to opioid receptors in the brain and spinal cord, blocking the transmission and processing of pain signals. They provide strong pain relief but carry a higher risk of dependence.
- Non-opioid analgesics (e.g., paracetamol/acetaminophen): Act primarily via central mechanisms that are not yet fully understood and lack significant anti-inflammatory effects.
- Co-analgesics (e.g., anticonvulsants, antidepressants): Not primarily pain medications, but they can exert analgesic effects in specific pain conditions, especially neuropathic pain.

Classification According to the WHO Pain Ladder
The World Health Organization (WHO) developed a three-step framework for pain management that guides the selection of analgesic agents:
- Step 1: Non-opioid analgesics (e.g., paracetamol, ibuprofen, diclofenac) – for mild pain
- Step 2: Weak opioids (e.g., tramadol, codeine) – for moderate pain
- Step 3: Strong opioids (e.g., morphine, oxycodone, fentanyl) – for severe to very severe pain
Side Effects and Risks
Depending on the substance class, analgesic medications may cause different side effects:
- NSAIDs: Gastric irritation, peptic ulcers, kidney damage, increased cardiovascular risk with long-term use
- Paracetamol: Severe liver damage in cases of overdose
- Opioids: Constipation, nausea, respiratory depression, dependence, tolerance development
- Co-analgesics: Dizziness, drowsiness, dry mouth (depending on the specific agent)
Non-Pharmacological Analgesic Approaches
In addition to medications, several non-pharmacological methods also provide analgesic effects and can be used as complementary or alternative strategies:
- Physiotherapy and exercise therapy
- Acupuncture
