What is Amitriptyline?
Amitriptyline is a medication belonging to the class of tricyclic antidepressants (TCAs). It has been used in clinical medicine since the 1960s and remains one of the most widely prescribed antidepressants worldwide. In addition to treating depression, amitriptyline is commonly used today for chronic pain management and certain sleep disorders.
Mechanism of Action
Amitriptyline works by inhibiting the reuptake of the neurotransmitters serotonin and noradrenaline (norepinephrine) in the nerve cells of the brain. This means these chemical messengers remain available in the synaptic cleft for longer, improving communication between nerve cells. This mechanism underlies both its mood-lifting and pain-relieving effects.
Amitriptyline also has antihistaminergic (sedating) and anticholinergic properties, which can be therapeutically beneficial but also contribute to certain side effects.
Indications
- Depression: Treatment of moderate to severe depressive episodes
- Chronic pain: Neuropathic pain, fibromyalgia, tension headaches, and migraine prevention
- Sleep disorders: Low-dose use to improve sleep quality
- Bedwetting (nocturnal enuresis): In selected cases in children above a certain age
Dosage and Administration
Amitriptyline is typically taken orally as a tablet. The dosage depends on the indication and the individual response of the patient:
- For depression: Usually 75–150 mg per day, in some cases up to 300 mg
- For chronic pain: Significantly lower doses, often 10–75 mg per day, usually taken in the evening
- For sleep disorders: Low doses of 10–25 mg before bedtime
Amitriptyline should always be taken as prescribed by a healthcare professional. Discontinuation should always be done gradually under medical supervision to avoid withdrawal symptoms.
Side Effects
Amitriptyline can cause various side effects, especially at the beginning of treatment:
- Common: Dry mouth, drowsiness, dizziness, constipation, weight gain, blurred vision
- Occasional: Cardiac arrhythmias, low blood pressure upon standing (orthostatic hypotension), urinary retention
- Rare: Liver dysfunction, seizures, hypersensitivity reactions
Elderly patients are particularly sensitive to the anticholinergic and cardiovascular side effects of amitriptyline and require close medical monitoring.
Contraindications and Interactions
Amitriptyline must not be taken in cases of:
- Hypersensitivity to the active substance
- Acute myocardial infarction or severe cardiac arrhythmias
- Narrow-angle glaucoma (elevated intraocular pressure)
- Concurrent use of MAO inhibitors (risk of severe interactions)
Interactions occur with other centrally acting substances, alcohol, antihypertensive medications, and certain cardiac drugs. A complete list of current medications should always be discussed with the treating physician.
Important Notes for Patients
The full therapeutic effect of amitriptyline in depression often takes two to four weeks of regular use to become apparent. Stopping the medication without medical advice can lead to relapse and withdrawal symptoms. If suicidal thoughts or a significant worsening of mood occur, medical help should be sought immediately.
