What Does Peroxisomal Mean?
The term peroxisomal is an adjective describing anything that relates to peroxisomes. Peroxisomes are small, membrane-bound organelles found in virtually all eukaryotic cells – that is, in the cells of animals, plants, and fungi. They play a central role in various metabolic processes and are essential for the health of the human body.
Structure and Function of Peroxisomes
Peroxisomes are spherical organelles with a diameter of approximately 0.1 to 1.0 micrometers. They contain a wide variety of enzymes that catalyze biochemical reactions. The most important peroxisomal functions include:
- Beta-oxidation of fatty acids: In particular, very long-chain fatty acids (VLCFAs) are broken down in peroxisomes before the products are further processed in the mitochondria.
- Synthesis of plasmalogens: Peroxisomes are involved in the production of certain phospholipids that are important for the function of nerve cells and cardiomyocytes.
- Detoxification of hydrogen peroxide: Peroxisomes both generate and neutralize hydrogen peroxide (H2O2) using the enzyme catalase, thereby preventing cell damage caused by reactive oxygen species.
- Cholesterol and bile acid metabolism: Peroxisomes are involved in the biosynthesis of bile acids and the processing of cholesterol.
Peroxisomal Disorders
Defects in the function or formation of peroxisomes lead to so-called peroxisomal disorders (also known as peroxisomal biogenesis disorders or peroxisomal metabolic diseases). These are mostly genetic in origin and primarily affect the nervous system, liver, and kidneys.
Peroxisomal Biogenesis Disorders
In these conditions, the formation of functional peroxisomes is broadly impaired. The best-known example is Zellweger syndrome, which causes severe neurological deficits, muscle weakness, and liver problems in infants. It belongs to the Zellweger spectrum, which also includes neonatal adrenoleukodystrophy and infantile Refsum disease.
Single Peroxisomal Enzyme Defects
In these disorders, the function of a single peroxisomal enzyme is impaired. A well-known example is X-linked adrenoleukodystrophy (X-ALD), in which very long-chain fatty acids cannot be broken down and accumulate in the nervous system and adrenal glands.
Diagnosis of Peroxisomal Disorders
Diagnosis is typically established through a combination of:
- Blood tests (e.g., elevated levels of very long-chain fatty acids)
- Imaging studies such as brain MRI
- Molecular genetic testing to identify gene mutations
- Enzymatic assays performed in specialized laboratories
Treatment of Peroxisomal Disorders
Causal treatment options remain limited. Therapeutic approaches include:
- Dietary interventions: Restriction of very long-chain fatty acid intake and supplementation with Lorenzo's oil (a mixture of glyceryl trioleate and glyceryl trierucate) in X-ALD.
- Stem cell transplantation: In certain forms of X-ALD, bone marrow transplantation can slow disease progression.
- Symptomatic therapy: Physical therapy, speech therapy, nutritional support, and treatment of adrenal insufficiency.
- Gene therapy: Newer research approaches aim at genetically correcting defective peroxisomal genes.
