What Is Chronic Pain?
Chronic pain is defined as pain that lasts for more than three months, either continuously or in recurring episodes. Unlike acute pain, which serves as a warning signal alerting the body to injury or illness, chronic pain often loses this protective function. It can persist even after the original cause has been resolved and is recognized as a disease in its own right.
Chronic pain affects hundreds of millions of people worldwide and is one of the most common reasons for medical consultations, disability, and reduced quality of life.
Causes
Chronic pain can arise from a wide variety of causes. While underlying physical conditions are often involved, psychological and social factors also play a significant role. The main types include:
- Nociceptive pain: Caused by ongoing tissue damage, such as in osteoarthritis, rheumatoid arthritis, or back disorders.
- Neuropathic pain: Caused by damage or dysfunction of the nervous system, such as in diabetic neuropathy, post-herpetic neuralgia, or herniated discs.
- Nociplastic pain: Caused by altered pain processing in the central nervous system without clear tissue damage, as seen in fibromyalgia or irritable bowel syndrome.
- Psychologically influenced pain: Conditions such as depression, anxiety disorders, and trauma can amplify or perpetuate pain.
Symptoms and Impact
Chronic pain can manifest in many ways and is often accompanied by a range of additional complaints:
- Persistent or recurring pain in one or more areas of the body

- Physical examination: Neurological and orthopedic tests to identify the source of pain.
- Imaging studies: X-ray, MRI, or CT scans to detect structural abnormalities.
- Psychological assessment: Standardized questionnaires to evaluate anxiety, depression, and pain-related thought patterns.
- Pain diaries: Patient-kept records of pain intensity and patterns to guide treatment planning.
Treatment
Treating chronic pain is complex and usually requires a multimodal approach that addresses physical, psychological, and social dimensions.
Medication
Depending on the type and cause of pain, various medications may be used:
- Non-opioid analgesics: Paracetamol, ibuprofen, or diclofenac for mild to moderate pain.
- Opioids: For severe pain, such as cancer-related pain or advanced osteoarthritis – only under strict medical supervision.
- Antidepressants and anticonvulsants: Particularly effective for neuropathic pain (e.g., amitriptyline, pregabalin, duloxetine).
- Topical preparations: Lidocaine patches or capsaicin creams for localized neuropathic pain.
Physical Therapy and Exercise
Regular physical activity and targeted physiotherapy are key components of pain management. They improve mobility, strengthen muscles, and can positively influence pain perception.
Psychological Therapy
Cognitive Behavioral Therapy (CBT) is one of the most effective psychological interventions for chronic pain. It helps patients identify and change unhelpful thought patterns and avoidance behaviors. Relaxation techniques such as progressive muscle relaxation, mindfulness-based stress reduction (MBSR), and biofeedback are also commonly used.
Interventional Procedures
In certain cases, interventional treatments may be beneficial:
- Nerve blocks and epidural injections
- Spinal cord stimulation
- Sympathetic nerve blocks for specific pain syndromes
Multimodal Pain Therapy
Multimodal pain therapy combines pharmacological, physical, and psychological treatment approaches within an interdisciplinary team. It is the preferred approach for complex chronic pain conditions and is carried out in specialized pain centers or outpatient clinics.

Early treatment of acute pain can help prevent chronification. The following strategies can also support the management of chronic pain:
