- Tumor diagnostics or removal: When joint tumors or synovial sarcomas are suspected
- Foreign body removal: After penetrating injuries involving foreign objects
Surgical Procedure
Arthrotomy is typically performed under general anesthesia or regional anesthesia. After sterilizing the skin, a precise surgical incision is made over the affected joint. Muscles, ligaments, and the joint capsule are carefully divided or retracted layer by layer to gain access to the joint cavity. Once the diagnostic or therapeutic measures are completed, the joint is meticulously closed by suturing all layers in anatomically correct order. A sterile dressing is then applied.
Arthrotomy vs. Arthroscopy
Compared to arthroscopy, a minimally invasive procedure using small incisions and a camera, arthrotomy is an open surgical technique requiring a larger incision. Arthroscopy is preferred whenever technically feasible, as it is associated with fewer complications, less blood loss, and faster recovery. However, arthrotomy remains indispensable when complex or extensive procedures cannot be performed minimally invasively.
Risks and Complications
As with any surgical procedure, arthrotomy carries certain risks:
- Wound or joint infections
- Post-operative bleeding or hematoma
- Nerve or vascular injury
- Scar tissue formation and restricted range of motion
- Deep vein thrombosis or pulmonary embolism, especially after lower limb procedures
- Reactions to anesthesia

Careful post-operative management is essential for a successful recovery after arthrotomy. Key elements include:
- Immobilization: The affected joint may be immobilized or kept non-weight-bearing for a defined period depending on the procedure.

- Wound care: Regular monitoring of wound healing and timely dressing changes.
- Pain management: Medication to relieve post-operative pain and support recovery.
The duration of rehabilitation depends on the type and extent of the procedure as well as the joint involved and may range from a few weeks to several months.
