What is the Cerebral Aqueduct?
The cerebral aqueduct – also known as the Aqueductus cerebri or Aqueductus mesencephali – is a narrow, tube-shaped canal located in the midbrain (mesencephalon). It connects the third ventricle to the fourth ventricle and serves as a critical passageway within the brain's internal fluid system. Cerebrospinal fluid (CSF) flows through this canal, providing nourishment, cushioning, and protection to the central nervous system.
Anatomy and Function
Measuring approximately 15 mm in length with a diameter of only 1–2 mm, the cerebral aqueduct is one of the narrowest passages in the CSF pathway. It runs through the center of the midbrain and is surrounded by the periaqueductal gray matter (PAG), a region known to play an important role in pain modulation and autonomic functions.

Clinical Significance: Disorders of the Aqueduct
Aqueductal Stenosis
The most common disorder affecting the cerebral aqueduct is aqueductal stenosis – a narrowing or complete blockage of the canal. This condition can be congenital (present from birth) or acquired due to inflammation, tumors, or scar tissue. Stenosis prevents normal CSF drainage and leads to hydrocephalus (an accumulation of fluid in the brain), causing increased intracranial pressure.
Obstructive Hydrocephalus
A blocked aqueduct results in so-called obstructive hydrocephalus. CSF can no longer flow from the third to the fourth ventricle, causing the ventricles to enlarge and intracranial pressure to rise. Without treatment, this can result in serious neurological damage.
Symptoms of Aqueduct Disorders
Symptoms related to aqueduct dysfunction are largely associated with increased intracranial pressure caused by hydrocephalus. Common symptoms include:
- Headaches, often worse in the morning
- Nausea and vomiting
- Visual disturbances (e.g., double vision)
- Gait disturbances and balance problems
- Cognitive impairment or changes in consciousness
- In infants: abnormal enlargement of the head circumference
Diagnosis
Imaging of the cerebral aqueduct is primarily performed using Magnetic Resonance Imaging (MRI). Specialized MRI sequences can visualize both the anatomical structure and CSF flow through the aqueduct. Computed Tomography (CT) may also be used to detect ventricular enlargement or signs of increased intracranial pressure.
Treatment
Treatment depends on the underlying cause of the aqueduct disorder:
- Endoscopic Third Ventriculostomy (ETV): A minimally invasive surgical procedure that creates a new opening in the floor of the third ventricle, bypassing the obstruction.
- Shunt Placement: Insertion of a catheter system (shunt) to divert excess CSF from the brain to another body cavity where it can be absorbed.
- Treatment of the Underlying Cause: If a tumor or infection is responsible, the primary condition is addressed accordingly.
