What is Cradle Cap?
Cradle cap, medically known as infantile seborrheic dermatitis, is a very common and benign skin condition affecting newborns and young infants. It appears as yellowish-white, greasy-looking scales or crusts on the scalp. Despite its appearance, cradle cap is entirely harmless, not contagious, and does not typically cause any itching or discomfort to the baby.
Causes
The exact cause of cradle cap is not fully understood, but the following factors are believed to contribute:
- Overactive sebaceous glands: Shortly after birth, maternal hormones may still be present in the baby, causing the oil-producing glands in the skin to become overactive.
- Malassezia yeast: The naturally occurring yeast Malassezia (formerly Pityrosporum) on the skin may play a role in triggering scale formation when sebum production is excessive.
- Hormonal influence: Maternal hormones passed during pregnancy or through breast milk can stimulate the baby skin glands.
Symptoms
Cradle cap typically presents with the following features:
- Yellowish-brown or whitish, greasy scales on the scalp
- Thick crusts that may adhere firmly to the scalp
- Mild redness of the affected skin areas
- Occasional spread to eyebrows, ears, or other areas of the face
- Generally no itching or pain for the infant
Diagnosis
Cradle cap is usually diagnosed by a visual examination performed by a pediatrician. No special tests are required. It is important to distinguish cradle cap from other skin conditions such as atopic dermatitis (eczema) or psoriasis, which can also cause scalp scaling but often involve significant itching and different distribution patterns.
Treatment
Cradle cap is often self-limiting and resolves on its own within the first few months of life. If treatment is desired, the following approaches are available:
Gentle Home Care
- Regular, gentle washing of the scalp with a mild baby shampoo
- Applying baby oil or olive oil to the affected areas, allowing it to soak in briefly, then carefully combing with a soft brush to loosen scales
- Avoid forcibly picking or scraping off the scales, as this may irritate the skin
Medical Treatment
- For persistent cradle cap, a pediatrician may recommend mild keratolytic creams or gentle antifungal shampoos (e.g., containing ketoconazole)
- Topical corticosteroids are rarely used and only in severe cases under medical supervision
When to See a Doctor
Parents should consult a doctor if:
- The scaling spreads extensively beyond the scalp
- The skin becomes severely inflamed, weeping, or shows signs of infection
- The baby appears clearly uncomfortable or scratches persistently
- A bacterial superinfection is suspected
