What Is Irritable Bowel Syndrome?
Irritable bowel syndrome (IBS) is a chronic functional disorder of the gastrointestinal tract. People with IBS experience recurring symptoms such as abdominal pain, bloating, and changes in bowel habits — including diarrhea, constipation, or both — without any detectable structural or organic abnormality such as inflammation or a tumor. IBS is one of the most common gastrointestinal conditions worldwide, affecting an estimated 10–15% of the global population.
Causes and Risk Factors
The exact causes of IBS are not yet fully understood. Research suggests that multiple factors interact to trigger the condition:
- Gut-brain axis dysfunction: Disrupted communication between the brain and the gut plays a central role in IBS.
- Visceral hypersensitivity: People with IBS have a lower pain threshold in the gut, making them more sensitive to normal digestive processes.
- Altered gut motility: The intestines contract too quickly or too slowly, leading to diarrhea or constipation respectively.
- Microbiome imbalance: Changes in the composition of gut bacteria (the microbiome) may contribute to IBS development.
- Psychological factors: Stress, anxiety, and depression are closely linked to IBS and can trigger or worsen symptoms.
- Post-infectious IBS: IBS can develop following an acute gastrointestinal infection.
- Genetic predisposition: A family history of IBS suggests a genetic component.
Symptoms
IBS symptoms vary widely between individuals and can fluctuate over time. Common symptoms include:
- Recurring abdominal pain or cramping, often relieved by a bowel movement
- Bloating and a feeling of fullness
- Changes in bowel habits: diarrhea, constipation, or alternating episodes of both
- Mucus in the stool
- A sensation of incomplete bowel emptying
Symptoms often occur in flares and may be triggered or worsened by certain foods, stress, or hormonal fluctuations. General symptoms such as fatigue, headaches, or back pain may also accompany the condition.
Diagnosis
There is no single definitive test for IBS. Diagnosis is based on the internationally recognized Rome IV criteria, which require that recurrent abdominal pain occurs at least one day per week over the last three months and is associated with at least two of the following: improvement with defecation, change in stool frequency, or change in stool appearance.
To rule out other conditions with similar symptoms, a doctor may order:
- Blood tests (e.g., inflammatory markers, coeliac disease antibodies, thyroid function)
- Stool tests for infections or inflammatory markers (e.g., calprotectin)
- Colonoscopy to exclude inflammatory bowel disease or colorectal cancer
- Abdominal ultrasound
Treatment
IBS is generally not curable, but symptoms can be effectively managed. Treatment is individualized based on the predominant symptom pattern.
Diet and Lifestyle
- Low-FODMAP diet: Reducing fermentable carbohydrates has proven effective for many IBS patients in reducing symptoms.
- Regular meals, adequate fluid intake, and a fiber-rich diet (for constipation-predominant IBS) are commonly recommended.
- Regular physical activity supports gut motility and reduces stress levels.
Medication
- Antispasmodics (e.g., hyoscine butylbromide) help relieve abdominal cramping.
- Laxatives or fiber supplements for constipation-predominant IBS.
- Antidiarrheals (e.g., loperamide) for diarrhea-predominant IBS.
- Probiotics may help restore gut microbiome balance.
- Low-dose antidepressants are used in some cases to modulate gut-brain signaling.
Psychological and Complementary Therapies
- Cognitive behavioral therapy (CBT) and stress management techniques address psychological triggers effectively.
- Gut-directed hypnotherapy has demonstrated significant benefit in clinical studies.
- Relaxation techniques such as yoga and mindfulness meditation can improve overall quality of life.
When to See a Doctor
While IBS is not a dangerous condition, certain warning signs require prompt medical evaluation:
- Unintentional weight loss
- Blood in the stool
- Symptoms that wake you from sleep at night
- Fever combined with abdominal pain
- New onset of symptoms after age 50
